• Recommended Immunization Schedule - Birth - 18 years. For a schedule in Spanish, click here.

• Claim an exemption from immunization requirements by following the directions on the Department of State Health Services website.

Parents can request, in writing, their child's physical fitness assessment results at the end of the school year.  To do this, please write to:
